Shared Care initiatives have demonstrated positive results in improving the coordination of care between family physicians, specialists, and other health professionals, in a variety of different ways.

CBT Spread Initiative
The Cognitive Behavioural Therapy (CBT) Skills Group program is an eight-week group medical visit (GMV) for adult patients (17.5-75 years old) that integrates basic neuroscience concepts with introductory mindfulness and CBT skills and teaching. The mentorship model of the GMVs allows for physicians to learn along with the patients, and to progress to a facilitator role if desired.
Partners in Care/Transitions in Care
Supports family physician/specialist-led improvement projects that encourage engagement, exploration of root issues, and stakeholder collaboration to improve the flow of care between providers and care settings.
Physician Leadership Development
Offers financial support with tuition, travel and accommodation for family physicians for leadership and quality improvement training through accredited training organizations.
Child and Youth Mental Health and Substance Use Community of Practice
The Community of Practice aims to equip doctors with the tools, skills, and relationships to advance child and youth mental health care, and substance use prevention and care across BC.
Rapid Access to Consultative Expertise (RACE)
Increases access to specialist advice for primary care providers through the RACE telephone line, mobile app and secure messaging.
Perinatal Community of Practice
Supports and unites physicians, midwives, and perinatal care providers in BC by equipping them with tools, skills, and resources while fostering collaboration and knowledge sharing—all aimed at advancing culturally safe and optimal patient care.
Teledermatology
Provides electronic access to dermatology consultation for primary care providers – reducing long wait times and the burden of travel for dermatology care.
